Client Activity Tracking Dashboard

We wish to create a website that allows our clients to track their employees task completion progress throughout the workday by clicking on tasks and entering appropriate data. We have a model that we have created a video of to provide an idea of what we are looking to accomplish. The first part shows the employee entry screens and the second part shows the management screens. There is a brief pause between the two sections. We would need the software setup where we can create Unique User Accounts for each client and be able to have a safe and secure site for the data. We would need the company to arrange the hosting solution and maintain the site so we will need to understand how that would work. We would like to start with a simple working prototype with some sample data to ensure we are on the right track and then keep building using an agile methodology. The company has to be available to communicate with us during the Eastern U.S. timeline and be able to provide working examples of similar work that they have accomplished. We would like each entry to provide a general estimate as to what the entire project would cost along with the estimated timeline. We would like our User Interface to be unique to our brand and to not look like the example we provide for the project. We will not need to provide sample data since the site accumulates the data as the user clicks on the tasks. The selected company must be able to communicate easily in U.S. English.
