Need to create an online registration form page to be added to an existing website from a PDF form The user will need to fill in the required fields in the form and upload 1 photo and there will be mandatory fields. At the end the user will click on submit and it will redirect him to a payment process page that needs to be created The user will pick one of 3 options 1: Bank transfer that will display bank details 2: Via direct credit card / master card / visa 3: pay pal Then once payment is confirmed an e-mail will be send to the users e-mail address that was submitted into the form with a thank you and confirmation message The form then will be send to an e-mail address for processing
