Login-, Register-, Data-entry page. php, jquery (ajax) ,css, mysql - repost

3 pages. Register, Login, Data-entry 1.) Register (username, password, verify password) via jquery validation and a call to a mysql database (ajax) to see if email, username already is in use) Notification on the right of the fields (ok symbol if ok, or not ok symbol with text same line) e.g 'E-mail already in use', Username already in use..Password to short min. ..not match ) Variable for min password, username length. Submit button (with validation, if ok jump to data-entry form) 2.) Login (via username password or email password) if validates go to data-entry page, password retrieve link 3.) Data-entry just general, 2 radio, 2 checkbox, textarea, textfield, DOB (dropdown day,month year), submit button should be written to mysql via ajax (no screen refresh) a symbol(ok) and text should be display 'data saved' --------------------------------------------------------------------------------------- The CSS should be like/similar as niceforms ([login to view URL]) Pure CSS no tables. Realization needs to be top notch, explanation on top, every line of code with explanation/comment, good naming convention. It should be able to be used as a show case.
