php pwd protection script

Hello The script must protect some php pages and allow access only to users that insert a password to a form. All passwords will be in a mysql db. The script must create passwords on request. I will input how many password to create, how many digit they will be and the script will create random passwords and list them on a writable textbox. Another button will allow me to add the created passwords to database When a password is used from user it will be deleted from db. The form where to insert password can be in the same domain and even in a different domain of the protected pages. The access to the protected pages will be time limited, ie 60 minutes. I will have the ability to change this value on the script configuration. If the user input a wrong password it will be redirected to an error page. When the time limit is exceeded the user will be redirected to another page. The script will handle a simple referer system. I only need to insert on db the referer id. For example if user comes to the form page with this url [login to view URL] the script will add a new record with value 1. If the user is redirected for time limit exceeded it will have the same referer id if it input a new password.
