Preoperative Testing Guidelines

I need to be able to ask the questions on the attached Excel file. (Will send when I get your contact info). Ideally, I’d like to do it on a form that has a little more pizzazz (radio buttons for “yes?? or “no??, etc) but keep it professional looking. There may be a better way of doing this than Excel, because people can type all kinds of responses in the cells, and it makes it harder to interpret the data. A nurse will call patients and fill out the data on the form via during an over-the-phone interview. Because the completed form will never be transferred over the internet, I am not sure that I need this data encrypted. The real key is the formula for the boxes under “Preoperative Testing Recommendations?? (57B and 57C). The answers to the questions on the form drive the resulting recommendations. If you just want to solve the problem quickly, here is what I need: The “Resting 12-Lead EKG?? should be performed 1)If procedure is Intermediate Risk, and patient has at least one of the clinical risk factors. 2)If procedure is intermediate risk and patient has at least one of the three following clinical risk factors: History of ischemic heart disease, History of TIA or stroke, or Peripheral Arterial Disease. 3)If procedure is High Risk 4)In all other instances, the EKG does not need to be performed. The recommendations for Noninvasive Stress Testing Before Noncardiac Surgery: 1)Obtain for patients with any of the Active Cardiac Conditions 2)Obtain for patients with 1 or more Clinical Risk Factors, poor functional capacity (less than 4 METs) undergoing intermediate or high risk surgery. I would also like to have some mechanism to make sure that all questions are answered (i.e., all responses are required responses), because an omission could result in the wrong preoperative testing if handled as a negative response. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows?
