291415 Multi-page Form/Survey Changes

We have an existing multi-page "wizard" style application form that collects information (much like a survey), provides a printable summary and stores the information in a MySQL database. Please ask in PMB for URL along with more details of the changes. We need to make the following improvements: a) Add a "back" button to the system. Currently all information is stored in the $_SESSION variable - when back is clicked, the previous page should be shown with the previous information entered, radio buttons selected etc. b) One of the select fields needs to be pulled from the database, with a small script to add/remove options c) There are a number of additional fields (some text, some radio buttons, some checkboxes) that need to be added to the form and the database d) A couple of pages need some JavaScript logic. For example a page that has "choose up to 3 checkboxes" needs to have JavaScript to prevent more than 3 being checked e) An additional summary page with “Accept” button, giving the user the opportunity to go back to correct any mistakes We are looking for someone who is tidy with their coding, will suggest changes or improvements to make the system work better, and can start straight away. The UI is in Spanish (not any coding, database field names, or comments) - so if you know Spanish, it's a plus but in no way necessary. Thanks for your bids!
