PHP Foursquare Suggest API and JQUERYUI Autocomplete

I need a basic php page (supported by jquery and any additional required .js files) containing a text box that will provide autocomplete options with foursquare api suggestions ('query' parameter = text entered in the box). [login to view URL] Autocomplete suggestions should be displayed in a vertical drop down list below the text box when user stops typing for x seconds. The suggestions should contain the 'name' on one line and 'address' on another line of the venue from the foursquare suggest API results. The autocomplete suggestions should be clickable with the cursor. Upon click, they populate the following form fields with information from the Foursquare API for the selected venue: - Original Visible Text Box: Name of Venue - Hidden Field: Category - Hidden Field: Address - Hidden Field: City - Hidden Field: State - Hidden Field: Postal Code - Hidden Field: Lat - Hidden Field: Lng The autocomplete needs to be configured based upon two URL parameters configuring the Foursquare Suggest API (obtained through PHP $_GET). - near - limit Thus, I would like to load and configure the page by calling the following URL: URL: [login to view URL],tx&limit=10 Examples of similar UI functionality: [login to view URL] Please provide URL of similar work in the past for evaluation.
