Site and logo design

I have a site at http://[[login to view URL]][1] and I want a new design template and logo for it. Something more clean and simple like <[login to view URL]> The template design must be your own original work and 100% royalty free. Design should consist of the following: width: 760 header left column - 150 pixels wide body right column - 150 pixels wide footer I want the following navigation buttons: Home About Us Products Survey Contact Us Logo should be hi-res in EPS format. Logo should also be incorporated in the header of the site design. Colors: blues, grays and white I want the code for the site to be created with Server Side Includes to break out the header, left column, body, right colum and footer.
