workorder software

I need a software that will act as a service work order software for a computer repair shop, where i'm able to print work orders; there should be customer's copy and workshop's copy. Must be able to input the following into a workorder: -Date/Time -Company Name -Contact Name -Address -Phone#s -Email -website ------ then 2nd section should have the following fields for input: -Equipment Type -username -password -current warranty: Parts / Labour / Parts+Labour / None - (if warranty exists) is original Invoice attached? Yes / No ------ then 3rd section should have the following field for input: -symptoms, problems or work to be done: -Regular Service / Express Service (if Express Service was selected, I want "WorkShop" copy to say "EXPRESS EXPRESS EXPRESS EXPRESS" in **BOLD** at the top and bottom of the workorder page. ------ one click printing should be defaulted to both "Customer" copy and "WorkShop" copy at the sametime, unless changed by the user. when workorder printed, it should have a decent looking design where all the above is printed on both the "Customer" copy and the "WorkShop" copy, as well as the company logo/name/address/phone#s and the workorder#. the following fields should be included on the "WorkShop" copy only: -Work Done: (leave enough space for writing the details of the repairs done) -Date Work Done: -Date Customer was called: -Tech Name: -(a semi-invoice at the end of the page which will have 5 columns; Qty, Code, Item, Price, Total. and at the bottom it should have the Subtotal, Gst, Total) -Signature: ------ read my text file inside that zip file, it will have more details on afew more things. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Microsoft Windows 2000 (all versions) Microsoft Windows XP (all versions) Microsoft Windows Vista (would be a bonus, but I can wait for a future upgrade to Vista, when Vista actualy gets released for public) Microsoft Windows 9x (not a must, as it is rarely used) Microsoft Windows ME (not a must, as it is never used)
