Design Two Dynamic Web Forms(repost)

I need 2 dynamic forms designed/coded. Please see the attached documents which show two forms. (1) Enquiry Form. This is an enquiry form for a student to fill in if they want to book a tutor. (2) Tutor application form. This is a form for a tutor to fill in if they would like to join our teaching team. They're pretty much complete in terms of fields, although I may need to add some more explanatory text. We might need to move some fields around as well when we see this designed and do some usability tests as well. In the docs I've made areas where user needs to enter something in Green, text on the page in Red and explanatory text for you in Black. Bold green means field is mandatory). The forms should be dynamic with new fields being shown when certain questions are answered. On submitting form then user should be shown fields which are mandatory and need to be filled in if they are not. I think it would also be good for the user to be able to review all information submitted before final submission. Design wise then this sort of style form looks good [login to view URL] but I'm open to ideas. Something modern and web 2.0. The forms need to blend in to the websites they will appear on. There are two websites where both forms need to go: (1) [login to view URL] (2) <[login to view URL]> This is a sister site currently in development. The 2 forms also need to go on here as well and fit in with the theme. While both the sites are developed in Wordpress I need these 4 pages designed as standalone pages in HTML. The responses from these forms will be fed into a backend system currently being developed by someone else. You may need to speak with them at some point which I can arrange.
