Build a custom calculator

I would like to have a custom calculator built that is web accessible. This calculator is used to determine if an invoice for medical records is compliant with state statutes. The inputs will be the number of pages (integer), the state (Lookahead picklist), the invoice amount (Dollar amount with two decimal places). The rules per state are in the attached file. The output display should indicate if the input invoice amount is acceptable by highlighting the calculator green if yes and red if no as well as displaying the maximum acceptable invoice amount based on the statutes. The variables for the state statutes are the number of pages including price per page based on tiers (Pages 1-50 are$1, page 51-100 are $.50 and pages 100+ are $.25) or flat rates such as each page is $.10. There may be caps per state such as the maximum amount is $25. There may be a base rate in addition to a per page such as $5 + .$25 per page. There may be shipping charges. All of these variables will be detailed in a spreadsheet with entries for each of the states.
