Osticket customization (users -> organizations)

Hi I'm using osticket v1.10 and I need some customization done to the Agent panel -> Users- -> Organizations search and display. What we've already configured (using osticket Manage List and Manage forms features) is that each Organization has the following additional data field: Zone (drop down list), row (number), level (number), unit number (number) (see my attachments). Now what we need is: 1. When adding a new organization, the name will be automatically filled in using the following format Zone Row Level-Unit. For example user chooses Zone as Somerset, row = 01, level = 02, unit = 22 => the Name field will automatically be filled with "Somerset Row 1, #02-22". If row is not filled in => "Somerset #02-22". Then user can submit that form. (see my attachment) 2. Everytime user updates this org record, name field will be modified accordingly, using the above rule as well. I highly prefer that this customization happens at the UI level, e.g. just some javascript to auto fill Name field whenever Zone/Row/Level/Unit field is edited in the form. Deliverables: only files that you've made changes to (and I expect it to be just 1-2 files). Please do NOT send us the whole codebase of osticket.
