Add fields in database

I own an adult cam site and company and would like to add a few fields in some areas. Two: I want a token counter/goal counter for models to set a goal for an action of some sort, and the tips to automatically get counted until she reaches her goal (e.g. [login to view URL]) and I want to add a "Social Security Number" field (and to the right in text "If you'll be working in the USA and you make over $600 dollars we need to send you a 1099 every year. A fake Social Security Number could result in problems with the IRS, for you!." If the person I hire is cheap, quick and efficient, I could almost guarantee more work in the near future. Please do not waste my time bidding if you cant work on this in the office because its an adult website or have any trouble understand English, I'm on a timeline here. Also, andriod and ios app developers are favored as this cam site has a mobile and app version to it as well.
