Opt-in page website with auto-submitter

I am looking to design a simple one page website which has the following features: 1-Fully customizable opt-in form to receive subscribers information and store it on a data base with the ability to export the subscriber information in different formats such as Excel, Access, text (data columns separated by comma or pipe) to be used with email softwares. 2-Opt-in Data forwarder, at the administrative-end of the web site, it must be possible to specify as many secondary opt-in URLs as possible, to AUTOMATICALLY submit the subscribers information (obtained from the primary opt-in page) to the secondary opt-in pages (the secondary opt-in pages are opt-in forms on separate websites than the initial website) 3- exit forwarder, as the visitor tries to close the webpage, the visitor should be redirected to secondary pages (to be specified at the Administrative-end of the website)
