We have a form that we want to be able to add to any website, and it must be able to add a file attachment and send it to the email address of our choice. I would prefer it in HTML, but PHP would be OK as a second choice. We have the HTML form we want to use, attached, and at the bottom area of the form we want to add the ability for the user to select a file from their computer and attach it, and both the email and the attachment get sent to any email address I add. ALSO, I WANT THE CODE SIMPLIFIED, SO ALL I HAVE TO DO IS INSERT THE CORRECT DOMAIN NAME, AND EMAIL ADDRESS ON THE FORM, AND ANY OTHER FILES NEEEDED TO BE ASSOCIATED WITH IT TO MAKE IT RUN. I do NOT want the form to have to undergo intensive changes to have it run. It will go on a linux type server, and I just want to substitute the domain name, and the delivery email address in the form code to any files, and I'm done. The form will reside in the main directory, no sub directory. This should be SIMPLE for an expert like you! thanks for your consideration. I have attached the form we want to use, with editor comments where we want your code and the attach file info boxes to go.

I am not reading the requirement as i feel I do not need to till you want me to. I know PHP, Mysql, javascript , can work on it. I can add the form, process file attachment and related things. the rest is just one of the many different requirements.
