Say, for example, "at the top of your lungs" literally means nothing in spanish (well, maybe it means something about running fast and getting short of breath), but there're another dozen of phrases that get the original idea to the reader (in neutral spanish, latin-neutral spanish or Spain dialect, you make a pick)
